  My first thought was that this might be related to a novel called "Purge" by Sofi Oksanen. At least judging from the background colour and the decorations in this picture, which match the covers of the book exactly. In the book a fly is a central symbol related to a woman being raped and forced to betray her family during the Soviet occupation of Estonia. The woman mentally escaped the violence by imaginning herself as a fly that is just sitting on the ceiling, seeing it happen to her, but not actually experiencing it. Maybe this woman is a rape victim..?
